For Medicaid in Ohio, is Medicaidcoverage based on time of service, or the date of submission of the bill?

So, I currently have Medicaid, but there is a possibility I could lose it in the future due to a change in circumstances. However, I also have a surgery coming up on October 1st, and I know Hospitals often take their sweet time billing and submitting claims, etc. So my question is, is Medicaid coverage based on the date I received care, or the date that the actual bills are submitted by the hospital, etc.? As in, if I received surgery while covered by Medicaid on October 1st, and lost my coverage on November 1st, would the bills still be covered because I had active Medicaid coverage and was eligible for it at the time?

I assumed that to be how it would work, l the info I could find online suggested that to be the case, but when I called the Medicaid hotline, they said I will be responsible for all bills received after losing coverage, even if the date of service fell under my coverage. Needless to say, I'm pretty confused, as I am not even aware of any other health coverage that works like this, but is that actually accurate for Medicaid in Ohio?
